The Per Housing Program provides housing support and referrals for individuals experiencing homelessness who face complex challenges such as mental health disorders, substance abuse, or chronic medical conditions. It serves a wide range of vulnerable populations, including those re-entering from the Department of Justice, participants in the Coordinated Entry System (CES), chronically homeless individuals, emancipated youth, and people referred by Drug Courts or state and county agencies. Specialized assistance is also offered to men diagnosed with HIV/AIDS, unhoused Veterans, and seniors, connecting them with the appropriate housing, treatment, or healthcare services to support long-term stability.
