Please include the following on your email: FULL NAME (and business name if you are a business), ADDRESS, REPORT NUMBER (If you do not have the report number, please give a brief description of the incident to assist us in locating the report), and DATE and approximate time of the incident. You will receive a response with in 5 business days of receipt of the request.
